Instructions for the (Losing) Registrant
- Prepare an Affidavit along the lines of the Form available on the next page. The language of affidavit may vary according to your local Court of Law, however, you need to state that – you owned the domain name(s) previously and you did not wish to get these domain names Transferred to Registrar <Gaining Registrar>
- Get this Affidavit notarized by the Notary Office of your local Court of Law.
- Enclose Photo Identification Proof of the Losing Registrant and his/her Contact Details, in the form of either -
a) Passport, or
b) Valid Driver’s License, or
c) Article of Incorporation, or
d) Military ID, or
e) State/Government issued ID, or
f) Birth Certificate
Before me, the undersigned notary public, this day ___________ (Date), __________________________ (Losing Registrant Name) appeared,who being duly sworn according to law, deposes the following:
I/We __________________________ (Losing Registrant Name) hereby confirms that the domain name(s) __________________________________________________ ____________ was/were registered to __________________________________________ (Losing Registrant Full Name) of the Company _____________________________ (Losing Registrant Company Name) until ___________________ (Date of Transfer Away to Registrar <Gaining Registrar>).
I/We __________________________ (Losing Registrant Name / Company Name) further affirms that I/we did not wish to Transfer our domain name(s) to Registrar <Gaining Registrar> from our Registrar <our Registrar Name> on _________________ (Date of Transfer Away to Registrar <Gaining Registrar>).
